If you are interested in making a change or want to know what your options could be, please fill out the Medicare and Prescription Drug Check-up Form and return for a no cost consultation. The form can be mailed, scanned and emailed or faxed, or take a picture on your phone and text.
Once submitted, we'll get together for a no cost consolation and go over all you plan options. We can meet either via zoom or in person at the Gahanna or Newark office. |
